– Application letter for the issuance of forms stating
the following;
• Full Names
• Address
• Phone Number
• Two Passport Photographs
• The Email Address of each Administrator (Minimum of Two and
Maximum of Four Administrators).
– Photocopy of Means of Identification of each
Administrator.
– Photocopy of Death Certificate or National Population
Commission Death Certificate of the Deceased. Original copy
produced for sighting or an affidavit of Loss of Death Certificate
and Police Report.
– Applicant to Swear to an Affidavit that no other application
has been previously made in either Lagos or Ikeja Registry.
– Forms to be submitted at the Head of Department Probate.
